APS Individual Medical Orders

These documents, previously known as the “Diabetes Management Plans” outlines the consented medical management prescribed for this individual student with Type 1 Diabetes while under the care and supervision of school or pre-school personnel. These medical orders cannot be altered by a third party without parental consent.
